To request the use of the PoTS UK logo and/or images for any of your events we require you to complete this form.

• PoTS UK logo is the property of the PoTS UK charity and should not be used without our consent
• PoTS UK logo can only be used to recognise events or activities of PoTS UK, or to promote the charity’s aims (providing consent has been given).
• The use of the PoTS UK logo does not imply that the charity endorses any planned activities, campaigns, products, or sponsors
